PHACE offers robust advocacy to ensure patients’ wishes and rights are respected throughout their healthcare journey. Our personal healthcare advocates coordinate with all providers to manage chronic and acute illnesses, fostering collaboration and communication. We ensure providers are aware of and adhere to patients’ preferences regarding service access, treatment options, and declination of care, promoting a patient-centered approach.

When additional information related to a diagnosis or treatment is needed, PHACE healthcare advocates step in to provide the necessary education or facilitate advocacy with the appropriate medical or mental health provider. This support spans various topics, including treatment options, care plans, healthcare rights, and other relevant areas, ensuring patients feel confident and informed.

Patients receive assistance in scheduling and managing referrals, appointments, and treatments. PHACE advocates can also provide virtual support during medical appointments, helping patients address concerns, obtain education, and seek clarification. Post-appointment, advocates are available to review and discuss the information, ensuring full understanding and satisfaction with care.

For patients undergoing inpatient care, PHACE facilitates collaboration between providers and advocates for patient rights and wishes. Support includes participating in discharge planning meetings to address follow-up care, coordinating post-hospitalization treatments and services, and arranging in-home care, outpatient services, or transportation as needed.
